3. State picture I.D., preferably a driver’s license.
4. Two (2) forms of payment:
a. 1 check made out to U.S. State Department (no cash)
b. 1 check made out to U.S. Postal Service (or cash) for processing fee.
5. Completed form DS-11 (passport application)
6. If applying for children under age 14 both parents must be present.
7. If applying for children 14 and over one parent must be present.
